Milken Scholars Program - Application Checklist

Your completed Milken Scholars Program application package will consist of:

	1.	A Five-page Application You may complete the application online by logging in with the username and password provided. If for some reason, you need a copy of the application, it is available as an Acrobat (pdf) download on this Website.
	2.	Family Financial Profile This form should be completed by or with your parents and submitted along with your application online. If you would rather mail it along with your other materials, you may download it as part of the application.
	3.	Personal Statement This may be completed online or mailed with your application materials to the Milken Scholars Program.
	4.	Two Recommendation Forms Your recommendation forms may be completed online by the individuals providing them. Or, they may give them to you in sealed envelopes and you can mail them along with your application materials to the Milken Scholars Program.
	5.	A copy of your current high school transcript Please mail the transcript along with your application materials to the Milken Scholars Program.
	6.	Copies of Your SAT I (or ACT) and SAT II Score Reports Please mail the copies of your score reports along with your application materials to the Milken Scholars Program. We do not require copies from the College Board. Instead, please photocopy the report that was sent to your home.
